Benefit Eligibility
Full time regular employees working 32 hours per week are eligible for all employee benefits and programs. Unless otherwise noted, benefits commence from date of hire.

Medical, Dental and Vision Care Plans

Medical Plan
California Micro Devices offers two health care plan options: Great West PPO and HMO. Refer to plan documents for more details.

Dental Care Plan
Indemnity Plan with United Concordia. You have a choice of going to a United Concordia provider or any dentist of your choice. There is an annual maximum of $1,500 for 2005. The deductible is $100 per calendar year. You can find a United Concordia covered dentist by going to www.ucci.com.

NOTE: A negotiated rate is offered on dental procedures if you go to a dentist within the United Concordia group, which could be lower than a dentist of your choice.

Vision Care Plan
Provided through VSP. Eye exam: 1 each 12 months, Prescription glasses: 1 pair each 12 months.

Prescription Plan
Provided through Express Scripts. If you have routine maintenance medication, this mail order prescription plan gives you the advantage of getting a 90 day supply of medicine for the price of a 60 day supply.
